Health Equity

NJ 211’s Health Equity initiative, in partnership with the NJ Department of Health, aims to address the disproportionate impact of COVID-19 and other health disparities on vulnerable and medically underserved communities, including people 65+. Since 2021, this project has connected over 7,100 Essex County residents with referrals to resources to boost their social determinants of health. The project now serves twenty high-need municipalities: Camden City, Newark, East Orange, Irvington, Bayonne, Hoboken, Jersey City, North Bergen, Union City, West New York, Edison, New Brunswick, Perth Amboy, Woodbridge, Trenton, Passaic City, Paterson, Franklin Township, Elizabeth, and Plainfield.
